Polypharmacy is a major risk factor for adverse drug events (ADEs), particularly when drug-drug and drug- gene interactions are insufficiently considered in routine care. The EU-funded SafePolyMed project addresses this challenge by developing a digital Medication Management Center (MMC) that integrates pharmacological knowledge, pharmacogenomic information, patient-reported outcomes, and advanced AI-based decision support. This paper presents the first deployed version of the SafePolyMed MMC, focusing on its service-oriented architecture, core functional components, and data protection design. The MMC supports polypharmacy patients to proactively manage their own healthcare while increasing transparency of treatment and raising alerts about health risks with focus on drug-drug interactions and actionable drug-gene interactions based on clinical guidelines. The system is designed as a modular, interoperable platform to support the future integration of AIbased risk prediction of ADEs as well as mechanistic precision dosing models.
