Systematic review compares cardiovascular impacts of obesity hypoventilation syndrome and isolated obstructive sleep apnea, suggesting tailored management strategies.
This systematic review compares the cardiovascular impact of obesity hypoventilation syndrome (OHS) and isolated obstructive sleep apnea (OSA), with a focus on pulmonary hypertension, cardiac remodeling, rhythm disturbances, and clinical outcomes. A systematic search was conducted across PubMed, Scopus, LISA, and Campbell Collaboration (2000–2025). OHS was consistently associated with greater right ventricular pressure overload, diastolic dysfunction, atrial fibrillation, nocturnal bradyarrhythmias, and higher mortality compared with isolated OSA. Long-term PAP therapy and intensive weight reduction were associated with improved cardiovascular proxy measures. The review argues for dedicated cardiorespiratory risk stratification and multidisciplinary management in this population.
