Obesity has emerged as a chronic, relapsing, multi-system disease that extends far beyond excess body weight, influencing metabolic, cardiovascular, renal, hepatic, reproductive, and functional health. In parallel, the therapeutic landscape has evolved rapidly, with GLP-1 based therapies particularly semaglutide, reshaping the management of obesity and its related co-morbidities through effects that extend from weight reduction and glycaemic improvement to broader organ protection by improving metabolic health. This compilation brings together contemporary scientific perspectives on obesity as an adiposity-based chronic disease and examines the role of semaglutide across diverse but interconnected clinical settings, including type 2 diabetes, cardio-kidney-metabolic syndrome, MASLD/MASH, women’s health, patient communication, real-world evidence, and longterm treatment support. Collectively, these chapters emphasize a shift from BMI-centric and glucose-centric care toward a complication-based, risk-stratified, and patient-centred model of management. The purpose of this work is to provide clinicians with a practical and evidence-informed resource that integrates evolving guideline recommendations, emerging trial data, and real-world considerations relevant to Indian practice. I hope that this supplement “Clinical Expert Insights on the Utility of GLP-1RA in Metabolic Health: Focus on Semaglutide” will support more meaningful, individualized, and outcome-focused obesity and metabolic health care in routine clinical practice. With warm regards, Dr. J.P.S.
