Key result
T2DM is not linked to aneurysm diameter or clinical presentation at AAA repair.
Why the study?
Type 2 diabetes mellitus has been paradoxically associated with a lower prevalence of abdominal aortic aneurysm, but its effect on aneurysm morphology and presentation remains unclear.
Does Type 2 diabetes mellitus affect aneurysm diameter or mode of presentation in patients undergoing infrarenal abdominal aortic aneurysm repair?

Does Type 2 diabetes mellitus affect aneurysm diameter or mode of presentation in patients undergoing infrarenal abdominal aortic aneurysm repair?
Type 2 diabetes mellitus does not independently affect aneurysm diameter or presentation mode at the time of infrarenal AAA repair, suggesting previously observed differences may be due to detection bias or presentation patterns.
No association between T2DM and AAA size or presentation at repair; leaves open diabetes effects on progression in broader populations.
Background Type 2 diabetes mellitus (T2DM) has been paradoxically associated with a lower prevalence of abdominal aortic aneurysm (AAA), but its effect on aneurysm morphology and presentation remains unclear. Methods We conducted a retrospective analysis of 387 patients who underwent infrarenal AAA repair between 2008 and 2018. Patients were stratified by T2DM status. Baseline aneurysm diameter was assessed using preoperative computed tomography angiography. Associations between T2DM, aneurysm size, and mode of presentation were evaluated. Results T2DM was present in 49 patients (12.7%). In unadjusted analyses, patients with T2DM had modestly smaller aneurysm diameters compared with non-diabetic patients. This association remained after adjustment for age, sex, and smoking status but was no longer statistically significant after accounting for clinical presentation. T2DM was not associated with the mode of AAA presentation. Conclusions T2DM was not independently associated with aneurysm diameter or clinical presentation at the time of repair. Observed differences in aneurysm size may reflect differences in presentation patterns or detection rather than a direct effect of diabetes on aneurysm morphology.

Dimova et al. (2026) conducted an observational in Infrarenal abdominal aortic aneurysm (n=387). Type 2 diabetes mellitus vs. Non-diabetic patients was evaluated on Aneurysm diameter and mode of presentation. Type 2 diabetes mellitus (present in 12.7% of patients) was not independently associated with aneurysm diameter or clinical presentation at the time of infrarenal abdominal aortic aneurysm repair.
