Abstract A newly formed apical thrombus in a patient with prior anterior myocardial infarction resulted in simultaneous cerebral embolism and myocardial injury. Multimodality imaging was essential in revealing the embolic mechanism and establishing a diagnosis of cardioembolic MINOCA.
