Totally endoscopic repair resolves severe MR in a double-orifice mitral valve without inducing stenosis.
Why the study?
Double-orifice mitral valve is a rare congenital anomaly, and repair strategies for mitral valve prolapse in this setting remain technically challenging.
Case Report (n=1)
Totally endoscopic mitral valve repair is a feasible and effective minimally invasive strategy for treating severe mitral regurgitation in patients with a double-orifice mitral valve.
Supports feasibility of endoscopic repair in selected DOMV cases; hypothesis-generating and should not yet change practice.
Double-orifice mitral valve (DOMV) is a rare congenital anomaly. Repair strategies for mitral valve prolapse in the setting of DOMV remain technically challenging. We report a case of DOMV presenting with severe mitral regurgitation (MR) due to chordal rupture and annular dilation, successfully treated with totally endoscopic mitral valve repair. Regarding the surgical approach, preserving the fibrous bridge, we repaired a DOMV via an endoscopic approach with neochords and an edge-to-edge stitch for leaflet prolapse, and a flexible band for annular dilatation, achieving trivial MR without stenosis. This case demonstrates minimally invasive mitral valve repair for DOMV, in which the regurgitation was successfully controlled while preserving the fibrous bridge and avoiding mitral stenosis.

Omori et al. (2026) conducted a case report in Double-orifice mitral valve with severe mitral regurgitation (n=1). Totally endoscopic mitral valve repair was evaluated on Mitral regurgitation severity and stenosis. Totally endoscopic mitral valve repair successfully treated severe mitral regurgitation in a double-orifice mitral valve, achieving trivial regurgitation without stenosis.
