MitraClip TEER reduces SAM and optimizes cardiovascular profile in a patient with severe post-AVR LVOTO.
Why the study?
Systolic anterior motion and dynamic LVOTO are rarely reported after surgical AVR, and there is limited literature regarding intervention with TEER in post-AVR patients.
Case Report (n=1)
Mitral TEER may serve as an effective rescue therapy for acute, refractory systolic anterior motion and LVOT obstruction following surgical aortic valve replacement.
May offer bailout TEER for refractory post-AVR LVOTO; hypothesis-generating and requires prospective validation before wider use.
BACKGROUND: Systolic anterior motion (SAM) and dynamic left ventricular outflow tract obstruction (LVOTO) is a well-described phenomenon in hypertrophic obstructive cardiomyopathy and rarely reported after surgical aortic valve replacement (AVR). CASE SUMMARY: We present the first reported case of SAM and LVOTO developing intraoperatively after surgical AVR for severe bicuspid aortic valve stenosis necessitating mitral transcatheter edge-to-edge repair (TEER) with the MitraClip. Optimal titration of pharmacologic management was not sufficient to relieve LVOTO in our patient. WHY BEYOND THE GUIDELINES: Despite management following conventional guidelines, the patient remained in critical condition. TEER with the MitraClip led to a reduction in SAM and optimization of his cardiovascular profile. DISCUSSION: There is limited literature regarding intervention with TEER on post-AVR patients. TAKE-HOME MESSAGE: LVOTO can present suddenly, as in our case, necessitating a multispecialty approach to unconventional interventions to stabilize a critically ill patient.

Heller et al. (2025) conducted a case report in Severe bicuspid aortic valve stenosis with intraoperative SAM and LVOTO after surgical AVR (n=1). Mitral transcatheter edge-to-edge repair (TEER) with MitraClip was evaluated on Reduction in SAM and optimization of cardiovascular profile. Mitral transcatheter edge-to-edge repair with the MitraClip successfully reduced systolic anterior motion and optimized the cardiovascular profile in a patient with severe post-AVR LVOTO.
