A female neonate with rotavirus-associated leukoencephalopathy presented with fifth-day fits that progressed to status epilepticus but eventually resolved with anti-seizure medications.
Case Report (n=1)
Rotavirus infection should be considered as a cause of neonatal seizures and leukoencephalopathy, presenting with specific diffusion-weighted imaging patterns.
Neonatal seizures with hypocalcemia unresponsive to repletion may require maximized ASM; single case leaves open optimal evaluation and generalizability.
A bstract A female neonate presented on day 4 of life with multifocal clonic seizures subsiding with Inj. Levetiracetam. She had no preceding history of loose motions, fever, or vomiting. Ionic calcium was found to be low initially. Its correction did not provide any improvement. The seizure pattern proceeded to status epilepticus on day 6 which necessitated maximization of all anti-seizure medication. By day 8, the status epilepticus waned off. An magnetic resonance imaging (MRI) study of the brain done on day 12 of life showed diffusion restriction areas in bilateral subcortical white matter and corpus callosum with no corresponding signal intensity change in T2-weighted imaging. Stool was tested for Rotavirus antigen which was found to be positive. A follow-up MRI study done on day 19 of life showed gliotic changes along the white matter of frontoparietal lobes suggestive of sequelae of the disease. The seizure medication was tapered off and the child was discharged on syrup levetiracetam.

Aantriksha et al. (2025) conducted a case report in Rotavirus Associated Leukoencephalopathy (n=1). Anti-seizure medications (including Levetiracetam) was evaluated on Seizure control and MRI findings. A female neonate with rotavirus-associated leukoencephalopathy presented with fifth-day fits that progressed to status epilepticus but eventually resolved with anti-seizure medications.
