Key result
Group 3 and Group 1 pulmonary hypertension show similar RV strain when matched by sex.
Why the study?
Group 3 pulmonary hypertension patients have disproportionate right ventricular dysfunction compared to pulmonary arterial hypertension, prompting evaluation of how sex and pulmonary hypertension etiology modulate this dysfunction.
Does Group 3 PH etiology compared to Group 1 PAH disproportionately worsen right ventricular function within the same biological sex?
Cohort (n=220)
No
Does Group 3 PH etiology compared to Group 1 PAH disproportionately worsen right ventricular function within the same biological sex?
The disproportionate right ventricular dysfunction typically observed in Group 3 pulmonary hypertension appears driven primarily by a higher male predominance in this population rather than etiology-specific impairment when compared to Group 1 PAH.

RV strain interpretation across PH groups requires sex adjustment; leaves open etiology-specific effects independent of demographics.
Group 3 pulmonary hypertension (PH) patients have disproportionate right ventricular dysfunction (RVD) compared to pulmonary arterial hypertension. We evaluated how sex and PH etiology modulated RVD. Strain echocardiography showed no intrasex differences between PH types. Heightened RVD in Group 3 PH may be due to a greater male proportion.

Kazmirczak et al. (2023) conducted a cohort in Group 1 pulmonary arterial hypertension (PAH) and Group 3 pulmonary hypertension (PH) (n=220). Group 3 Pulmonary Hypertension vs. Group 1 Pulmonary Arterial Hypertension (PAH) was evaluated on Intrasex difference in right ventricular global longitudinal strain (RV GLS) between Group 3 PH and Group 1 PAH (females) (Mean difference (95% CI: -2.8% to 0.4%), 95% CI -2.8 to 0.4). Strain echocardiography revealed no significant intrasex differences in right ventricular function between Group 3 and Group 1 pulmonary hypertension, suggesting worse function in Group 3 overall is driven by a higher proportion of males.
