Telmisartan and azilsartan provide superior blood pressure lowering compared to other antihypertensive drugs, with telmisartan demonstrating proven efficacy on hard cardiovascular endpoints and better cost-effectiveness.
Why the study?
The renin-angiotensin-aldosterone system is a central pathogenetic mechanism across the cardiovascular continuum, warranting a review of the role and evidence for telmisartan and azilsartan in modern cardiology.
Do telmisartan and azilsartan improve blood pressure control and cardiovascular outcomes in patients with arterial hypertension compared to other antihypertensive drugs?
Do telmisartan and azilsartan improve blood pressure control and cardiovascular outcomes in patients with arterial hypertension compared to other antihypertensive drugs?
Telmisartan and azilsartan are highly effective for blood pressure reduction, but telmisartan is preferred due to its proven cardiovascular outcome benefits and better cost-effectiveness.
Telmisartan and azilsartan may aid BP control with organ protection; leaves open azilsartan's effects on hard endpoints.
The activity of the renin-angiotensin-aldosterone system is one of the main pathogenetic mechanisms underlying cardiovascular diseases at all stages of the cardiovascular continuum. This article discusses the role of telmisartan and azilsartan as the most powerful sartans in modern cardiology. Azilsartan and especially telmisartan have a significant organoprotection and are superior to other antihypertensive drugs in terms of lowering blood pressure. However, the effect of azilsartan on hard endpoints has not been studied while the efficacy of telmisartan on hard endpoints has been evaluated in plenty clinical trials including 3 large randomized clinical trials with several thousand patients. The article also presents calculations showing the better cost-effectiveness of telmisartan compared to azilsartan.

Zakiev et al. (2023) conducted a review in Arterial hypertension. Telmisartan and Azilsartan vs. Other antihypertensive drugs was evaluated. Telmisartan and azilsartan provide superior blood pressure lowering compared to other antihypertensive drugs, with telmisartan demonstrating proven efficacy on hard cardiovascular endpoints and better cost-effectiveness.
