SGLT2is added to standard therapy improve LA remodeling at 6 months in paroxysmal AF.
Why the study?
AF is among the most commonly encountered arrhythmias worldwide, and the effect of adding SGLT2is on left atrial remodeling and cardiovascular outcomes in nonvalvular paroxysmal AF needed evaluation.
Does the addition of SGLT2 inhibitors improve left atrial remodeling in patients with nonvalvular paroxysmal atrial fibrillation?

Does the addition of SGLT2 inhibitors improve left atrial remodeling in patients with nonvalvular paroxysmal atrial fibrillation?
Adding SGLT2 inhibitors to standard therapy improves left atrial structural and functional remodeling in patients with nonvalvular paroxysmal atrial fibrillation.
May support adjunctive SGLT2 inhibitor use in paroxysmal AF; leaves open confirmation in larger outcome trials.
Background: Atrial fibrillation (AF) was ranked among the most commonly encountered arrhythmia worldwide. Aims: It is aimed to evaluate the effect of adding sodium-glucose co-transporter-2 inhibitors (SGLT2is) on left atrial remodeling and cardiovascular outcome in nonvalvular paroxysmal AF. Patients and Methods: A total of 60 cases with nonvalvular paroxysmal AF were randomized into two equal groups. Echocardiogram was performed at baseline and 6-month follow-up. Results: After 6 months of treatment, left atrial diameter (LAD) and left atrial volume index (LAVI) were significantly decreased in Group A than Group B. In contrast, left atrium average strain was significantly improved in Group A than Group B. Moreover, LAD% and LAVI change either absolute or relative was significantly decreased in Group A than B. Furthermore, there was a significant improvement in the LA strain absolute and relative% change in Group A than Group B. Conclusion: The addition of SGLT2is to the present classical therapy in AF patients can improve left atrial remodeling.

Soliman et al. (2026) conducted an RCT in nonvalvular paroxysmal atrial fibrillation (n=60). Sodium-glucose co-transporter-2 inhibitors (SGLT2is) vs. Classical therapy was evaluated on Left atrial remodeling (left atrial diameter, left atrial volume index, and left atrium average strain). Adding SGLT2is to classical therapy in 60 patients with nonvalvular paroxysmal atrial fibrillation significantly improved left atrial remodeling, including LAD, LAVI, and LA strain, at 6 months.
