Overview

May warrant individualized AMS programming checks in AF; Level 5 case report leaves open prospective validation.

Structured PICO

Do varying atrial sensitivity, AV interval, and detection algorithms affect the efficacy of automatic mode switching in patients with pacemakers?

P
Population
18 patients (6 women; mean age 64 years) with pacemakers having automatic mode switching (AMS) capability who developed atrial fibrillation (n=14) or atrial flutter (n=4).
I
Intervention
Programming changes to pacemaker settings including varying atrial sensitivity, increasing AV intervals from 60 to 200 ms, and making detection algorithms more stringent (from 5 beats at 150 beats/min to 11 beats at 200 beats/min).
C
Comparator
Baseline pacemaker settings (normal sinus rhythm sensitivity, shorter AV intervals, less stringent detection algorithms).
O
Outcome
Efficacy of automatic mode switching (AMS) during atrial tachyarrhythmia.surrogate

Effective automatic mode switching in pacemakers during atrial fibrillation requires higher atrial sensitivity settings, shorter AV intervals, and less stringent detection algorithms.
