Small observational data hint at parallel carotid and LV hypertrophic remodeling in HT+DM; leaves open independent correlation and needs larger prospective validation.
Carotid wall thickening, plaque buildup and left ventricular hypertrophy (LVH) are markers of target organ damage in hypertensive patients (pts). The aim of the study was to investigate whether hypertrophy processes in carotid arteries (CA) and heart have the similar features according to the common CA (CCA) arterial mass (AM) and LV mass index (LVMI) in pts with hypertension (HT) and type 2 diabetes mellitus (DM). Methods: We study 30 pts (mean age 61±7 yrs) with carotid intima-media thickness (IMT)≥0.9 mm measured by ultrasound duplex scanning, divided into two groups: 1st – IMT 0.9<1.3 mm (n=18), 2nd – IMT≥1.3 mm (n=12). AM was calculated by formula: AM = ρ × L × (πRe2 − πRi2), were ρ is arterial wall density (ρ=1,06×103 kg/m2), L – arterial segment length (1 cm), Re – external, Ri – internal radius. CCA hypertrophy was defined as AM>0.275 g, eccentric/concentric type – according to IMT/diameter CCA ratio (IMT/dCCA) <0.20/≥0.20. By using 2D-echocardiography end-diastolic diameter (EDD), end-diastolic volume (EDV) and index (EDVI), LV ejection fraction, LVMI, relative wall thickness (RWT) were calculated. LVH was defined as LVMI >115 g/m2 in men, >95 g/m2 in women, eccentric/concentric type – RWT <0.42/≥0.42. Blood pressure (BP) monitoring, glycemic and lipid profile were also performed. Data are presented as M ± SD. Pearson's and Spearmen correlation analysis was used.
