Does a combined antihypertensive drug regimen including captopril improve severe hypertension in pregnant women?
This investigation suggested captopril was effective and safe for severe resistant hypertension in late pregnancy, a finding that strongly contradicts current clinical guidelines which strictly contraindicate ACE inhibitors during pregnancy due to fetal toxicity.
Should not change practice; hypothesis-generating for captopril in refractory late-pregnancy hypertension pending controlled trials.
The purpose of the present investigation was to evaluate a combined antihypertensive drug regimen including angiotensin converting enzyme inhibitor (captopril) in the treatment of hypertension in pregnancy. The combined use of captopril in late pregnancy produced a favorable therapeutic response to severe hypertension in pregnancy which was resistant to the treatment with other antihypertensive agents. There were no significant maternal, fetal and neonatal side-effects attributable to captopril when used in late pregnancy.
