Implication

Supports 14-day ticlopidine post-stenting in this cohort; leaves open randomized trials to confirm optimal duration.

Structured PICO

Does discontinuing ticlopidine after 14 days prevent stent thrombosis and reduce adverse events in patients undergoing successful intracoronary stent placement?

P
Population
827 patients undergoing successful stent placement in 1061 coronary segments at Mayo Clinic. Mean age 64, 49% prior MI, 20% prior CABG, 65% multivessel disease. Exclusions: chronic warfarin therapy, cardiogenic shock, and enrollment in research protocols requiring 4 weeks of ticlopidine.
I
Intervention
Ticlopidine discontinued after 14 days
O
Outcome
Adverse cardiovascular events (stent thrombosis, cardiovascular death, myocardial infarction, coronary artery bypass operations, or repeat angioplasty) between the 15th and 30th dayshard clinical

Discontinuing ticlopidine 14 days after successful intracoronary stent placement is safe and associated with a very low risk of stent thrombosis.
