Implication

May support functional gains with aortic valve reconstruction in select patients; leaves open durability and reoperation risk for prospective evaluation.

Structured PICO

Does aortic valve reconstruction improve valve function and prevent reoperation in patients with severe aortic valve disease?

P
Population
50 patients who underwent aortic valve reconstruction. Follow-up cohort of 46 patients (35 males, 11 females, mean age 39.1 years) primarily with rheumatic disease (n=27), infective endocarditis (n=16), or degenerative disease (n=3). Included isolated aortic valve disease (n=22), double-valve disease (n=16), and triple-valve disease (n=7).
I
Intervention
Aortic valve reconstruction (including subcommissural annuloplasty, cusp thinning, commissurotomy, free-edge unrolling, cusp extension with autologous pericardium, and aortic valve replacement with autologous pericardium)
O
Outcome
Aortic valve function improvement and need for reoperationhard clinical

Aortic valve reconstruction provides marked functional improvement in selected patients, though the need for reoperation remains a significant concern at midterm follow-up.

Limitations

  • reoperation remains an important problem
  • long-term follow-up is needed to assess the role of this operation
