Key result
In a 54-year-old male with mitral valve prolapse and a 9.3 mm mitral annulus disjunction, 4D XStrain speckle tracking echocardiography revealed significant reductions in global longitudinal and circumferential strain.
Why the study?
Mitral annular disjunction is easily identifiable on TTE but remains poorly recognized, despite being associated with mitral valve prolapse and posing risk for ventricular arrhythmias and sudden cardiac death.

4D XStrain speckle tracking echocardiography can detect early subclinical left ventricular systolic dysfunction in patients with mitral annulus disjunction and mitral valve prolapse.
Mitral annular disjunction (MAD) is an easily identifiable entity on transthoracic echocardiography, but is still poorly recognized or ignored. It is often associated with mitral valve prolapse and is itself a risk marker for ventricular arrhythmias and sudden cardiac death. We present here a case of 54 year Indian male presenting with MVP accompanied by significant mitral annulus disjunction (MAD) in whom a comprehensive transthoracic echocardiography (TTE) and 4Dimensional X Strain speckle tracking imaging was conducted to estimate the MAD distance and importantly, various LV strain parameters were evaluated to discover any early markers of LV aberration by LV strain imaging by 4Dimensional X Strain speckle tracking echocardiography (4DXStrain STE).

Mehrotra et al. (2024) conducted a case report in Mitral Annulus Disjunction and Mitral Valve Prolapse (n=1). Mitral annulus disjunction was evaluated on Left ventricular strain parameters. In a 54-year-old male with mitral valve prolapse and a 9.3 mm mitral annulus disjunction, 4D XStrain speckle tracking echocardiography revealed significant reductions in global longitudinal and circumferential strain.
