In men with acute heart failure, lower ejection fraction (p=0.008) and lower creatine clearance (p<0.001) were independent predictors of higher NT-proBNP values, whereas previous myocardial infarction was not.
Observational (n=87)
No
Effect estimate: ß=-0.544 for creatine clearance; ß=-0.272 for ejection fraction
p-value: p=<0.001
In men with acute heart failure, renal function and ejection fraction are stronger independent predictors of heart failure severity (NT-proBNP) than a history of previous myocardial infarction.
May refine NT-proBNP interpretation in men with acute HF by prioritizing renal function and EF; extends observational predictor data but leaves practice implications open.
The Goal: Processes of cardiac remodelling, especially collagen deposition and following fibrosis, seen both in infarcted and non-infarcted myocardium are important contributors to the development of the impaired left ventricular (LV) function. The objective of this study was to assess the independent influences of various clinical variables, including previous myocardial infarction (MI), on the severity of heart failure (HF) in men. Patients and Methods: This prospective study included consecutive male patients with acute HF, hospitalized at the Division of Cardiology, Department of Internal Medicine, University Hospital Centre Split. Statistical analysis was performed by means of T-test and linear regression in univariate, and multiple logistic regression in multivariate analysis. The P value of less than 0.05 was considered to be statistically significant.

Bušić et al. (2014) conducted an observational in Acute heart failure (n=87). Previous myocardial infarction, renal function, and ejection fraction was evaluated on Severity of heart failure measured by serum NT-proBNP values (ß=-0.544 for creatine clearance; ß=-0.272 for ejection fraction, p=<0.001). In men with acute heart failure, lower ejection fraction (p=0.008) and lower creatine clearance (p<0.001) were independent predictors of higher NT-proBNP values, whereas previous myocardial infarction was not.
