Implication

May support vascular risk stratification in nondiabetic HD patients; hypothesis-generating and requires prospective confirmation.

Structured PICO

P
Population
143 nondiabetic patients with end-stage renal disease (ESRD) undergoing maintenance hemodialysis for >6 months, mean age 63.9 years, 60% male, based in Japan. Excluded: severe cerebrovascular disease, severe PAOD with amputation/gangrene, severe joint deformity, noncontrolled neoplasm, decompensated liver cirrhosis, and severe congestive heart failure.
O
Outcome
Aortic pulse wave velocity (Ao-PWV) and ankle-brachial blood pressure index (ABPI)surrogate

In nondiabetic hemodialysis patients, HbA1c and hs-CRP are independent predictors of aortic wall stiffness and peripheral arterial occlusive disease, respectively, suggesting the utility of monitoring these markers.

Limitations

  • Strict patient selection criteria may have counteracted the influence of insulin resistance on arteriosclerosis
  • Cross-sectional design cannot determine whether elevated CRP is a cause or result of PAOD
  • Cannot determine if the duration of the uremic milieu of HD per se contributes to arterial alterations
