Key result
Adding low-dose alfentanil to nicardipine significantly attenuates post-intubation heart rate increases versus nicardipine alone.
Why the study?
The effect of combining nicardipine and low-dose alfentanil on the hemodynamic response following endotracheal intubation required evaluation.
Does a combination of low-dose nicardipine (10 μg/kg) and alfentanil (5 μg/kg) more effectively attenuate blood pressure and heart rate increases following endotracheal intubation compared to nicardipine (20 μg/kg) alone in ASA I-II adult patients?

Does a combination of low-dose nicardipine (10 μg/kg) and alfentanil (5 μg/kg) more effectively attenuate blood pressure and heart rate increases following endotracheal intubation compared to nicardipine (20 μg/kg) alone in ASA I-II adult patients?
Combining reduced-dose nicardipine (10 μg/kg) with low-dose alfentanil (5 μg/kg) blunts the reflex and catecholamine-induced tachycardia associated with nicardipine monotherapy during endotracheal intubation.
Supports lower-dose nicardipine plus alfentanil for intubation hemodynamics; extends randomized comparisons versus monotherapy in elective anesthesia.
Background: The present study was designed to examine the effect of a combination of nicardipine and low dose alfentanil on hemodynamic response following endotracheal intubation. Methods: Thirty patients (20-65 yr) were assigned randomly to receive nicardipine (N: 20μg/kg) or a combination of nicardipine and low dose of alfentanil (NA: nicardipine 10μg/kg and alfentanil 5μg/kg).Systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) were measured at 1, 2 min after anesthetic induction, and every minute for 5 min after intubation. Results: The magnitude of increases in SBP and DBP were non-significantly smaller in the NA group than in the N group, and increase in HR were significantly lower in the NA group. Conclusions:These results indicate that the nicardipine with alfentanil combination was more effective than nicardipine alone at attenuating blood pressure and heart rate increases following intubation.(Korean J Anesthesiol 2005; 48: 587∼91)

Kim et al. (2005) conducted an RCT in Hemodynamic response following endotracheal intubation (n=30). Nicardipine plus low-dose alfentanil vs. Nicardipine 20 μg/kg IV alone was evaluated on Hemodynamic response (systolic blood pressure, diastolic blood pressure, and heart rate) up to 5 minutes after endotracheal intubation (p=<0.05). Combining nicardipine (10 μg/kg) with low-dose alfentanil (5 μg/kg) significantly attenuated post-intubation heart rate increases compared to nicardipine (20 μg/kg) alone (p<0.05).
