May favor carvedilol for tolerability in low-H/M DCM patients; hypothesis-generating for MIBG-guided beta-blocker choice pending RCTs.
Does carvedilol improve tolerability, cardiac function, and neurohumoral factors compared to metoprolol in patients with dilated cardiomyopathy?
Carvedilol and metoprolol similarly improve cardiac function and neurohumoral profiles in dilated cardiomyopathy when tolerated, but carvedilol is significantly better tolerated in patients with severe sympathetic hyperactivity as indicated by a low delayed H/M ratio on MIBG imaging.
