Summary: By means of a prospective study, relapse after discontinuation of antiepileptic drug treatment in 433 children with epilepsy and 40 patients who were treated after the first seizure was investigated. Independent of the elec‐troencephalographic findings, the age of the patients, and other factors, the antiepileptic drugs were reduced during 1, 3, 6, and 12 months after 2, 3, and 4 seizure‐free years; and in children with absences after 1, 2, 3, and 4 seizure‐free years. The observation period after stopping therapy was at least 3 (on average, 5–6) years. In 157 of 433 children (36.3%) and 5 of 40 patients (12.5%), relapses occurred during this time. More than half (61.8%) of the relapses occurred during the withdrawal period or within 3 months: altogether, 86% within 1 year after discontinuation of therapy. Eighty‐six percent of these patients again became free from seizures on administration of the original therapy. The dependence on predictive factors of the rate of relapse was tested by multivariate statistical analysis. There was a pronounced significant dependence on the duration of the seizure‐free period, the duration of the withdrawal period, the length of illness, the frequency and duration of seizures, and the presence of paroxysmal activity in the EEG at the start of the discontinuation of antiepileptic drug treatment. The stopping of therapy during the pubertal period did not present a higher risk. ZUSAMMENFASSUNG Mit Hilfe einer prospektiven Untersuchung berichtet der Autor über Rezidive nach Beendigung der antiepileptischen Therapie bei 433 Kindern mit Epilepsie und 40 Patienten die nach dem ersten Anfall behandelt wurden. Unabhängig von den EEG‐Befunden, vom Alter der Patienten und anderen Faktoren wurden die Antiepileptika innerhalb 4 Wochen, 3, 6 und 12 Monaten, nach 2, 3, und 4 Jahren reduziert. Bei Kindern mit Absencen erfolgte die Reduktion nach einer anfallsfreien Periode von 1, 2, 3 und 4 Jahren. Die Beobachtungsperiode nach Beendigung der Therapie betrug wenigstens 3, im Durchschnitt 5 bis 6 Jahre. Bei 157 (36.3%) von 433 und bei 5 (12.5%) von 40 Patienten, erfolgten während dieser Zeit Rezidive. 61.8% der Rezidive ereigneten sich während des Absetzens oder innerhalb von 3 Monaten; zusammen 86% innerhalb eines Jahres nach Beendigung der Therapie. 86% dieser Patienten wurden nach Wiedereinführung ihrer ehemals genommenen Medikamente wieder anfallsfrei. Die Abhängigkeit der Rezidivhäufigkeit von Vorhersagefaktoren wurde durch die multivariate statistische Analyse untersucht. Es bestand eine ausgeprägte signifikante Abhängigkeit von der Dauer der anfallsfreien Periode. der Dauer der Zeit während des Absetzens, der Krankheitsdauer, der Häufigkeit und Anfallsdauer und der Gegenwart paroxysmaler Aktivität im EEG zu Beginn der Reduktion antiepileptischer Medikamente. RESUMEN Mediante un estudio prospectivo el autor publica las recaidas tras la interrupcion del tratamientó medicamentoso antiepilep‐tico en 433 niños epilépticos y en 40 enfermos tratados tras su primer ataque. Independientemente de los hallazgos en el EEG, las edades, y otros factores, las medicaciones se redujeron en cuatro semanas y tres, seis y doce meses tras dos, ties y cuatro años de tratamiento; en los niños con ausencias después de uno, dos, tres y cuatro años sin ataques. El periódo de observación después de la interrupción de la medicación duró, al menos, tres años, promediando entre cinco y seis años. En 157 (36.3 porciento) de los 433 y en 5 (12.5 por ciento) de los 40 enfermos, se produjeron recaídas durante el tiempo de observación. El 61.8 de las recaídas ocurrieron durante el periódo de reducción o en los tres meses siguientes y en el 86 por ciento de todos los casos en el primer ario despues de la interrupcion. El 86 por ciento de estos casos no tuvo nuevos ataques al volver a tomar las medicaciones originales. Los posibles factores que podrían hacer pensar en recaídas fueron estudiados con análisis estadísticos multivariados.
