A BSTRACT Hypertension remains one of the most important modifiable risk factors for cardiovascular morbidity and mortality worldwide. Numerous national and international guidelines have been released in recent years, each with varying recommendations on diagnostic thresholds, staging, treatment initiation criteria, and target blood pressures (BP). The major guidelines for managing hypertension from reputable organizations such as the American College of Cardiology/American Heart Association 2025, European Society of Cardiology 2024, European Society of Hypertension 2023, International Society of Hypertension 2020, and Indian Society of Hypertension 2023 are critically examined and contrasted in this narrative review. With an emphasis on their suitability in different clinical contexts, we highlight major similarities and differences in classification, treatment initiation strategies, first-line pharmacologic options, and BP targets. The review also outlines the Public Health Implementation Strategy, based on the Directorate General of Health Services Hypertension Treatment Protocol under the National Programme for Prevention and Control of Noncommunicable Diseases, which emphasizes practical methods for population-level hypertension management. Additionally, clinical implications of these guideline differences, recent evidence from landmark trials supporting lower BP targets, and the role of cardiovascular risk assessment are discussed. The review concludes with likely future directions, including digital health integration, renal denervation, and the need for more region-specific data from low-and middle-income countries. By synthesizing these guidelines and implementation strategies, this article aims to assist clinicians and policymakers in adopting evidence-based, context-appropriate approaches to optimize hypertension management and improve cardiovascular outcomes globally.
