The position of the transcatheter heart valve and the cusp-overlap view can be used to effectively guide left bundle branch area pacing in patients developing conduction disturbances after TAVR.
We illustrate a successful case of left bundle branch area pacing with contemporary transcatheter aortic valve replacement. The association between the position of the transcatheter heart valve and the cusp-overlap view may be used to effectively identify the targeted left bundle branch area after transcatheter aortic valve replacement.

Sato et al. (2025) conducted a case report in Severe aortic valve stenosis with post-TAVR complete AV block (n=1). Left bundle branch area pacing guided by cusp-overlap view was evaluated on Successful left bundle branch area pacing (pacing threshold and QRS duration). Left bundle branch area pacing was successfully performed using the transcatheter heart valve location and cusp-overlap view, achieving an excellent pacing threshold and a narrow paced QRS duration of 108 ms.
