Key result
A high cBIN1 score (≥ 1.9) predicted an increased 1-year risk of cardiovascular events (43% vs. 26%, HR 1.93) in ambulatory patients with HFrEF.
Why the study?
Although cBIN1 score (CS) diagnoses ambulatory heart failure and prognosticates hospitalization in HFpEF, its performance had not been tested in patients with HFrEF.
Does the cBIN1 score (CS) diagnose failing heart muscle and predict cardiovascular events in ambulatory HFrEF patients?
Cohort (n=273)
No
Does the cBIN1 score (CS) diagnose failing heart muscle and predict cardiovascular events in ambulatory HFrEF patients?
Hazard Ratio: 1.93 (95% CI 1.07–3.48)
Absolute Event Rate: 43% vs 26%
p-value: p=0.03
The cBIN1 score (CS) is a novel blood-based biomarker that successfully differentiates HFrEF patients from healthy controls and independently predicts 1-year cardiovascular events, independent of volume status.
May enhance risk stratification in ambulatory HFrEF; hypothesis-generating and requires prospective validation before clinical adoption.

Hitzeman et al. (2020) conducted a cohort in Heart failure with reduced ejection fraction (HFrEF) (n=273). High cBIN1 Score (CS ≥ 1.9) vs. Low cBIN1 Score (CS < 1.9) was evaluated on Cardiovascular events (HF or cardiac hospitalization, LVAD/MCS placement, orthotopic heart transplantation, or death) (HR 1.93, 95% CI 1.07-3.48, p=0.03). A high cBIN1 score (≥ 1.9) predicted an increased 1-year risk of cardiovascular events (43% vs. 26%, HR 1.93) in ambulatory patients with HFrEF.
