Hypertension in pediatric neuroblastoma can be challenging to manage due to its multifactorial etiology. We present a 3-year-old girl who presented with cardiogenic shock, echocardiographic evidence of dilated cardiomyopathy, right renal artery stenosis, and elevated catecholamines. Somatostatin receptor (SSTR) PET/CT revealed intense tracer uptake in a right adrenal mass and a low-grade diffuse myocardial uptake, suggestive of inflammatory myocarditis. This unusual finding highlights the potential role of SSTR PET/CT in detecting signs of myocarditis, with myocardial SSTR uptake as a possible biomarker of cardiac inflammation.
