1,639 adults (≥18 years) hospitalized for heart failure, hemodynamically stable, with LVEF ≤40%, elevated natriuretic peptides (BNP ≥400 pg/mL or NT-proBNP ≥1600 pg/mL), and signs/symptoms of fluid overload. Mean age 65, multinational (North and South America, Europe, Asia).

Intervention
Aliskiren 150 mg (increased to 300 mg as tolerated) daily, added to standard therapy, continued after discharge for a median 11.3 months.
