Fluid overload in patients with hematologic malignancies resulted in exaggerated NT-proBNP production, with values reaching tens to hundred thousands pg/mL.
Case Report (n=3)
Patients with hematologic malignancies may exhibit extremely elevated NT-proBNP levels in response to hypervolemia, potentially due to myocardial infiltration or assay interference.
May warrant caution interpreting extreme NT-proBNP as HF severity alone in hematologic malignancy; hypothesis-generating and requires prospective validation.
©2011 Wiley Periodicals Inc. Extremely elevated serum brain natriuretic peptide (BNP) in cancer patients is a poorly understood phenomenon. The authors report three cases of patients with hematologic malignancies and serial N-terminal pro-BNP (NT-proBNP) measurements with values in the range of tens to hundred thousands pg/mL. Through matching NT-proBNP results with clinical, laboratory, echocardiographic and radiologic data, the authors found that these patients demonstrated exaggerated responses to fluid overload. Patients with hematologic malignancies may have higher than expected values of NT-proBNP in response to hypervolemic states. The authors hypothesize that this may be related to possible infiltration of the myocardium by substances produced in the setting of these diseases or due to proteins interfering with the assay.Congest Heart Fail.

Andreu et al. (2011) conducted a case report in Hematologic malignancies (n=3). Fluid overload in hematologic malignancies was evaluated on NT-proBNP levels. Fluid overload in patients with hematologic malignancies resulted in exaggerated NT-proBNP production, with values reaching tens to hundred thousands pg/mL.
