Key result
Warm water irrigation and mediastinal dissection trigger intraoperative cardiac arrest, with recovery within ~2 minutes.
Why the study?
Reports on the frequency, mechanisms, and patient backgrounds of accidental intraoperative cardiac arrest during lung cancer surgery are lacking.
Case Report (n=4)
No
Sudden cardiac arrest can occur during lung cancer surgery due to vasovagal reflex or warm distilled water irrigation, but prompt intervention yields favorable outcomes.

Rapid response ensures recovery from rare intraoperative arrests in lung cancer surgery; leaves open need for prospective studies on triggers.
Vasovagal reflex and cardiac dysfunction due to intrathoracic warm distilled water are rarely but occasionally seen as complications of lung cancer surgery. However, there has been no report on the frequency, mechanism, or patient background of accidental cardiac arrest during surgery. We encountered four cases of sudden cardiac arrest without mechanical compression of the heart or catastrophic bleeding during surgery. One patient underwent mediastinal lymph node dissection, and three suffered cardiac arrest during flushing with warm distilled water in the thoracic cav-ity. In all four cases, the cardiac arrest time was within two minutes due to manual heart compression and natural re-sumption of spontaneous heartbeat, and no serious complications or sequelae occurred after the operation. However, one patient suffered cerebral infarction at seven days after surgery, although the infarction improved with heparin ad-ministration, and no sequelae were noted. Accidental cardiac arrest during surgery can occur during intrathoracic irrigation, but appropriate treatment with an anesthesiologist on hand can lead to a favorable course without any serious complications or sequelae.

Iwai et al. (2021) conducted a case report in Lung cancer undergoing surgery (n=4). Intrathoracic warm distilled water flushing or lymph node dissection was evaluated on Recovery from intraoperative cardiac arrest. Sudden cardiac arrest occurred in four patients during lung cancer surgery due to intrathoracic warm distilled water flushing or mediastinal dissection, with all recovering heartbeat within two minutes.
