To evaluate whether antithyroid pretreatment with methimazole impairs cure rates or delays the onset of hypothyroidism following radioactive iodine therapy in Graves' hyperthyroidism.
Randomized trial of 42 newly diagnosed Graves' hyperthyroidism patients allocated to radioiodine (131I) therapy with methimazole pretreatment (30 mg/day for ≥2 months, stopped 6 days prior) plus beta-blocker (n=21) or beta-blocker alone (atenolol 50-100 mg/day, n=21).
Eight patients were excluded post-randomization (5 pretreated, 3 nonpretreated), leaving 34 prospectively evaluated participants receiving a baseline 15 mCi 131I dose adjusted for goiter size and uptake.
All but one patient in each group developed hypothyroidism, taking 112 days (95% CI, 28 to 196) in the methimazole-pretreated group versus 106 days (95% CI, 45 to 167) in the beta-blocker alone group.
Free thyroxine normalized at 35 ± 30 days in pretreated patients compared with 44 ± 39 days in nonpretreated patients (p = 0.57), declining to subnormal levels at 65 ± 32 days versus 80 ± 70 days (p = 0.46).
