Key result
Preoperative diastolic dysfunction linked to ~2.3-fold higher risk of post-operative AF after off-pump CABG.
Why the study?
Post-operative atrial fibrillation is a common complication after coronary artery bypass grafting with associated increased risk of stroke, mortality, and hospitalization, but predictors such as diastolic dysfunction need evaluation.
Does preoperative left ventricular diastolic dysfunction predict post-operative atrial fibrillation in patients undergoing isolated off-pump coronary artery bypass grafting?
Observational (n=264)
No
Does preoperative left ventricular diastolic dysfunction predict post-operative atrial fibrillation in patients undergoing isolated off-pump coronary artery bypass grafting?
Odds Ratio: 2.26 (95% CI 1.23–4.15)
p-value: p=<0.01
Preoperative assessment of left ventricular diastolic function (E/e' > 8) is a significant predictor of post-operative atrial fibrillation following off-pump CABG.
May support preoperative diastolic assessment for post-op AF risk after off-pump CABG; hypothesis-generating, requires prospective validation.
Post-operative atrial fibrillation is a common complication after coronary artery bypass grafting. It is associated with increased risk of stroke and mortality, and duration of hospitalization. In 264 patients undergoing elective isolated off-pump coronary artery bypass grafting, preoperative clinical and echocardiographic data were compared between patients with and without post-operative atrial fibrillation. Diastolic dysfunction was defined as E/e’ 8 (E: Peak early mitral pulsed-wave Doppler flow, e’: mitral lateral annular early velocities). In this study, diastolic dysfunction and age were the independent risk factors of post-operative atrial fibrillation. The evaluation of preoperative diastolic dysfunction might be useful in identification of high risk patient of post-operative atrial

Nakahara et al. (2015) conducted an observational in Elective isolated off-pump coronary artery bypass grafting (n=264). Diastolic dysfunction (E/e' > 8) vs. No diastolic dysfunction was evaluated on Post-operative atrial fibrillation (OR 2.26, 95% CI 1.23-4.15, p=<0.01). Preoperative diastolic dysfunction independently increased the risk of post-operative atrial fibrillation after off-pump coronary artery bypass grafting (OR 2.26).
