Key result
In patients with atrial fibrillation and CHA2DS2-VASc ≥2, markers of thrombogenicity were higher in the right atrium compared to the left atrium, with no difference in inflammatory properties.
Why the study?
Recent trials found a poor temporal relationship between AF episodes and strokes, suggesting stroke in AF patients likely involves mechanisms beyond cardiac embolism.
Are there differences in markers of inflammation, thrombosis, and platelet reactivity between the left and right atria in patients with atrial fibrillation?

Are there differences in markers of inflammation, thrombosis, and platelet reactivity between the left and right atria in patients with atrial fibrillation?
In patients with atrial fibrillation and elevated stroke risk, thrombogenicity markers are higher in the right atrium than the left atrium, suggesting complex systemic or bi-atrial mechanisms of stroke beyond left atrial stasis.
Hypothesis-generating for bi-atrial thrombogenicity in high-risk AF; leaves open whether right atrial markers predict events or alter management.
BACKGROUND: Recent trials found poor temporal relationship between atrial fibrillation (AF) episodes and strokes. Thus, stroke in AF patients probably involves more mechanisms than cardiac embolism. We compared factors of inflammation, thrombosis and platelet reactivity between left (LA) and right atria (RA) and femoral vein (FV) in patients with AF. METHODS: Blood samples were collected from patients undergoing AF-ablation from the FV, RA and LA for neutrophil to lymphocyte ratio (NLR), immature platelet fraction (IPF) and count (IPC), CD40 ligand, P-selectin and E-Selectin. IPF was measured by an autoanalyzer; CD40 ligand, P-selectin, and E-Selectin were measured by ELISA and NLR was calculated from complete blood counts. RESULTS: -VASC≥2 but not in patients with low score. CONCLUSIONS: -VASc≥2, have higher markers of thrombogenicity in RA compared to LA. There was no difference in inflammatory properties between the atria.

A 2021 study conducted an observational in Atrial Fibrillation. Right atrial blood sampling vs. Left atrial blood sampling was evaluated on Markers of thrombogenicity and inflammation. In patients with atrial fibrillation and CHA2DS2-VASc ≥2, markers of thrombogenicity were higher in the right atrium compared to the left atrium, with no difference in inflammatory properties.
