Comprehensive pre-kidney failure education (CPE) delivered in a two-phase, intent-to-teach manner (interactive audio-visual education followed by individual counseling), either face-to-face or via telemedicine.

Comparator
Enhanced usual care (EUC) consisting of usual clinical care supplemented by printed hand-outs directing to freely available online self-learning CKD resources.

Proportion of Veterans using home dialysis at 90-days post-kidney failure.
