Staged surgical intervention can be successfully employed for extensive dissecting aortic aneurysms involving the entire aorta in patients with Marfan syndrome.
Staged repair may be feasible in Marfan patients with pan-aortic dissection; leaves open generalizability pending larger series.
広範囲に及ぶ解離性大動脈瘤や,病変が大動脈全体に及ぶMarfan症候群に対する根治手術は,これまできわめて困難であった.しかし,最近の外科手術の進歩により,分割手術を用いても積極的に人工血管置換手術が行われるようになってきた.われわれも最近,Marfan症候群に伴う,annuloaortic ectasia, DeBakey II型解離+右総腸骨動脈閉塞を伴うIIIb型解離性大動脈瘤の症例に対して,二期的に根治手術を行ったので報告する.症例は24歳男性で,初回手術として,II型解離と右総腸骨動脈閉塞に対し,Cabrol手術とilio-iliac bypass手術を施行し,約2年後に,拡大した遺残IIIb型解離性大動脈瘤に対して,下行大動脈起始部から左総腸骨動脈に至る全下行大動脈人工血管置換術を施行した.
