Why the study?
Primary splenic angiosarcoma is extremely rare with poor prognosis, especially when presenting with splenic rupture, and established treatments are currently lacking.
Primary splenic angiosarcoma presenting with rupture and intraabdominal hemorrhage has a very poor prognosis, with this case resulting in death at 106 days despite multimodal treatment.
Adds to sparse splenic angiosarcoma reports; leaves optimal multimodal therapy open pending prospective data.
症例は75歳,男性.眼前暗黒感,腹痛および呼吸苦を主訴に当院へ救急搬送となった.造影ctでは脾臓と肝臓に複数の腫瘍および血性腹水を認め,腫瘍破裂による腹腔内出血が疑われた.全身状態は保たれており,翌日に待機的に脾臓摘出および外側区域切除術を施行した.病理検査の結果,脾臓原発血管肉腫および肝転移と診断した.術後32日目と34日目に各々腹腔内出血を認め,各々経カテーテル動脈塞栓術(以下,tae)で止血を得た.十分な説明と同意のもと,2回目のtae施行後10日目に化学療法(パクリタキセル)を開始したが,1クール施行後のctにて肝転移の増大を認め,化学療法は中止した.その後dicを併発し,術後106日目に永眠した.脾臓原発血管肉腫は非常に希少かつ予後不良な疾患であり,特に脾破裂で発見された場合には平均予後は4.4カ月と報告されている1).治療も現状では確立されたものはない.若干の文献を混じえて検討を行ったので報告する.
