Transcatheter aortic valve implantation in a patient with an interventricular membranous septal aneurysm was complicated by aortic root injury and cardiac tamponade requiring emergency sternotomy.
Why the study?
The report presents the uncommon association between severe aortic stenosis and interventricular membranous septal aneurysm and discusses its impact on post-TAVI major complications.

The presence of an interventricular membranous septal aneurysm can complicate native valve crossing during TAVI, potentially increasing the risk of aortic root injury and cardiac tamponade.
May heighten perforation risk during TAVI with septal aneurysm; leaves open need for tailored imaging and access strategies.
An 86-year-old lady with severe aortic stenosis and interventricular membranous septal aneurysm underwent transfemoral transcatheter aortic valve implantation (TAVI). A balloon-expandable valve was deployed after a difficult native valve crossing. Transesophageal echocardiography showed a rapidly accumulating pericardial effusion, with pericardial thrombus and subsequent cardiac tamponade. The angiographic views raised suspicion of aortic root perforation. Median sternotomy was performed because of sudden hemodynamic collapse. The report presents the uncommon association between severe aortic stenosis and interventricular membranous septal aneurysm in an octogenarian and discusses its impact on the development of a post-TAVI major complication.

Dregoesc et al. (2020) conducted a case report in Severe aortic stenosis and interventricular membranous septal aneurysm (n=1). Transcatheter aortic valve implantation (TAVI) was evaluated on Procedural complications. Transcatheter aortic valve implantation in a patient with an interventricular membranous septal aneurysm was complicated by aortic root injury and cardiac tamponade requiring emergency sternotomy.
