Why the study?

The study evaluated the circadian efficacy of a telmisartan 40 mg/S-amlodipine 2.5 mg fixed-dose combination compared to telmisartan 80 mg in patients with essential hypertension not responding to 2-4 weeks of telmisartan 40 mg.

Does a fixed-dose combination of telmisartan 40 mg/S-amlodipine 2.5 mg improve circadian blood pressure compared to telmisartan 80 mg in patients with essential hypertension not responding to telmisartan 40 mg?

Population

217 Korean patients with essential hypertension unresponsive to 2-4 weeks of telmisartan 40 mg

Comparison

Telmisartan 40 mg/S-amlodipine 2.5 mg fixed-dose combination vs telmisartan 80 mg

Design

Randomized trial

Follow-up

8 weeks

Overview

Low-dose combination outperforms telmisartan dose escalation for 24-h BP control in non-responders; leaves open confirmation in larger diverse trials.

Structured PICO

Does a fixed-dose combination of telmisartan 40 mg/S-amlodipine 2.5 mg improve circadian blood pressure compared to telmisartan 80 mg in patients with essential hypertension not responding to telmisartan 40 mg?

P
Population
217 Korean patients with essential hypertension (clinic mean sitting systolic blood pressure [MSSBP] ≥140 mmHg, or ≥ 130 mmHg in those with diabetes mellitus or chronic kidney disease) who did not respond to 2-4 weeks' treatment with telmisartan 40 mg.
I
Intervention
Telmisartan 40 mg/S-amlodipine 2.5 mg fixed-dose combination for 8 weeks
C
Comparator
Telmisartan 80 mg for 8 weeks
O
Outcome
Changes in mean 24-h SBP and DBP on 24-h ambulatory blood pressure monitoring (ABPM) from baseline after 8 weekssurrogate

A fixed-dose combination of telmisartan and S-amlodipine is more effective than doubling the dose of telmisartan monotherapy for lowering 24-hour ambulatory blood pressure in patients with uncontrolled essential hypertension.
