Key result
Myval TAVI is linked to ~25% lower mean gradients versus Sapien 3 in large aortic annuli.
Why the study?
Does the Myval balloon-expandable valve improve in-hospital hemodynamics or clinical outcomes compared to the Sapien 3 valve in patients with large aortic annuli undergoing TAVI?
Observational (n=40)
No
Does the Myval balloon-expandable valve improve in-hospital hemodynamics or clinical outcomes compared to the Sapien 3 valve in patients with large aortic annuli undergoing TAVI?
Absolute Event Rate: 8.5% vs 11.4%
p-value: p=.04
In patients with large aortic annuli, TAVI with the Myval valve was associated with lower post-procedural echocardiographic gradients but a trend toward increased paravalvular leak compared to the Sapien 3 valve.

Alachkar et al. (2026) conducted an observational in Aortic stenosis with large aortic annuli (n=40). Myval valve vs. Sapien 3 valve was evaluated on Mean echocardiographic post-procedural gradient (Pmean) (p=.04). In patients with large aortic annuli, TAVI with the Myval valve was associated with significantly lower echocardiographic post-procedural mean gradients compared to the Sapien 3 valve (8.5 vs 11.4 mmHg, p=0.04).
