Early onset of a first cardiac allograft rejection episode was significantly associated with an increased frequency of multiple rejection episodes in the first 90 days post-transplant (P<.0001).

Does early onset of a first cardiac allograft rejection episode predict multiple rejection episodes in cardiac transplant recipients?
Patients presenting with an early onset of their first cardiac allograft rejection episode are at significantly higher risk for multiple rejection episodes within the first 90 days and require closer monitoring.

Early rejection may flag higher-risk recipients for closer surveillance; hypothesis-generating for prospective validation in cardiac transplant.
To determine the risk factors associated with development of acute cardiac allograft rejection during the first 3 months after transplant, data from 1,552 endomyocardial biopsies from 122 cardiac transplant recipients receiving their first graft were analyzed. An association (P< .0001) between the onset of a first rejection episode and the frequency of rejection episodes in the first 90 days was established. Our findings suggest that patients presenting with an early onset of first cardiac allograft rejection episode are at risk for multiple rejection episodes and should therefore be more closely monitored. Salom et al. (1995) conducted a cohort in Cardiac transplant recipients (n=122). Early onset of first cardiac allograft rejection episode vs. Later or no early onset of first rejection episode was evaluated on Frequency of rejection episodes in the first 90 days (p=<.0001). Early onset of a first cardiac allograft rejection episode was significantly associated with an increased frequency of multiple rejection episodes in the first 90 days post-transplant (P<.0001).
