This review highlights pulmonary endarterectomy (PEA) as the treatment of choice for operable CTEPH and riociguat as the first medical therapy for inoperable cases.
Chronic thromboembolic pulmonary hypertension (CTEPH) is a unique form of pulmonary hypertension (PH) arising from the obstruction of pulmonary arterial vessels by organised thromboembolic material [1]. Much like other forms of PH, CTEPH has historically proven to be a challenging clinical entity in that it is frequently underdiagnosed and undertreated [1–3]. This lack of clinical recognition can result in patients with CTEPH experiencing progressive PH and eventual right ventricular failure [1, 4]. PEA is the treatment of choice for operable CTEPH. Riociguat is the first medical therapy for inoperable CTEPH
