A combined atrioventricular synchronic leadless pacemaker and subcutaneous ICD system was implanted in a patient with bilateral upper limb venous occlusion.
Case Report (n=1)
Demonstrates the feasibility of combining a leadless pacemaker and subcutaneous ICD in a patient with bilateral upper limb venous occlusion and prior device infection.
Supports feasibility of combined leadless pacemaker-S-ICD in venous occlusion; leaves open need for prospective validation before wider use.
A 49-year-old female, with a history of hypertrophic cardiomyopathy, aborted cardiac sudden death, complete atrioventricular block, pacemaker syndrome, several previous interventions on the electronic devices, and bilateral subclavian vein thrombosis, was admitted to hospital due to infective endocarditis related to her dual-chamber implantable cardiac defibrillator (ICD). After complete removal of all the device’s components, and once antibiotic treatment had been completed, she underwent a single intervention to implant a combined atrioventricular synchronic leadless pacemaker (Micra AV, Medtronic®) and a subcutaneous ICD system (Emblem, Boston Scientific®). The pacemaker was placed in the mid-interventricular septum. The subcutaneous ICD generator was then implanted in the left lateral...

Fernández-Palacios et al. (2020) conducted a case report in Hypertrophic cardiomyopathy, complete atrioventricular block, bilateral subclavian vein thrombosis, infective endocarditis (n=1). Combined atrioventricular synchronic leadless pacemaker and subcutaneous ICD system was evaluated. A combined atrioventricular synchronic leadless pacemaker and subcutaneous ICD system was implanted in a patient with bilateral upper limb venous occlusion.
