Aorto-caval fistula from ruptured abdominal aneurysm is a fatal surgical emergency that can be diagnosed clinically by the presence of high-output heart failure and an expansible abdominal mass with thrill and bruit.
Prompts consideration of aorto-caval fistula in high-output failure with abdominal mass; case-level evidence leaves open prospective validation.
Two operated cases of aorto-caval fistula due to ruptured abdominal aneurysm are reported. This is a rare and real surgical emergency and is fatal without operation. The classical symptoms of high-output failure occurring simultaneously with the findings of an expansible abdominal mass with thrill and bruit should facilitate clinical diagnosis.
