Key result
Across 14 studies, transthoracic echocardiography and cine-CMRI showed high specificity, and contrast TTE had the highest sensitivity for detecting left ventricular thrombus compared to DE-CMRI.
Why the study?
Left ventricular thrombus after acute myocardial infarction carries significant embolic risk, requiring evaluation of common imaging modalities for early and accurate diagnosis.
Does TTE with or without contrast and cine-CMRI provide comparable diagnostic performance to DE-CMRI for detecting left ventricular thrombus in post-AMI patients?
Meta-Analysis
Does TTE with or without contrast and cine-CMRI provide comparable diagnostic performance to DE-CMRI for detecting left ventricular thrombus in post-AMI patients?
Contrast TTE offers high sensitivity and specificity for detecting left ventricular thrombus post-AMI, supporting its use as a primary imaging modality with CMRI reserved for inconclusive cases.
Supports contrast TTE as first-line for post-AMI LV thrombus detection; reinforces high specificity of TTE and cine-CMRI versus DE-CMRI.
Left ventricular thrombus (LVT) can occur in patients after acute myocardial infarction (AMI) and carries significant embolic risk. Leveraging imaging tools that can guide the early and accurate diagnosis of LVT and appropriate use of anti-thrombotic therapy can mitigate the risk of embolic events. This study aims to evaluate the utility of common imaging modalities for the diagnosis of LVT in post-AMI patients. Several databases (Medline, Embase and Cochrane Library) were searched until May 2025 for studies reporting on the comparative diagnostic performance of imaging modalities for LVT in post-AMI patients. Fourteen studies were identified for inclusion. Compared to delayed enhancement cardiac magnetic resonance imaging (DE-CMRI), transthoracic echocardiography (TTE) with or without contrast and cine-CMRI all had high specificity and contrast TTE had the highest sensitivity for LVT detection. A strategy to identify high-risk patients who would benefit from further imaging with CMRI to identify LVT should be considered for patients whose TTE is inconclusive. Low et al. (2026) conducted a meta-analysis in Left ventricular thrombus following acute myocardial infarction. Transthoracic echocardiography (TTE) and cine-CMRI vs. Delayed enhancement cardiac magnetic resonance imaging (DE-CMRI) was evaluated on Diagnostic performance (sensitivity and specificity) for LVT detection. Across 14 studies, transthoracic echocardiography and cine-CMRI showed high specificity, and contrast TTE had the highest sensitivity for detecting left ventricular thrombus compared to DE-CMRI.
