Key result
CRISPR-Cas-associated SCCmec variants in MRSA evaded rapid diagnostic detection, with 11 of 40 variants escaping detection by BCID2 or Xpert, representing 2% of 2,432 screened patients.
Why the study?
Sequence variation in SCCmec can disrupt primer binding in rapid molecular assays, yielding false-negative methicillin-resistant Staphylococcus aureus detection.

Do CRISPR-Cas-associated SCCmec variants in MRSA evade rapid diagnostic detection?
CRISPR-Cas-associated SCCmec variants in MRSA can evade rapid molecular diagnostic assays, posing a threat to diagnostic accuracy.
Clinicians should back up negative MRSA molecular assays with culture in suspected BSI; leaves open broader variant prevalence and assay redesign needs.
Rapid molecular assays guiding treatment of methicillin-resistant Staphylococcus aureus detect SCCmec (Xpert) or the SCCmec-orfX junction (BCID2). Sequence variation in this region can disrupt primer binding, yielding false-negative results. Investigation of a missed bloodstream infection linked escape to a CRISPR-Cas-associated SCCmec variant, leading to identification of 64 variants from 45 patients-2% of 2432 screened. Misdiagnosis was restricted to clonal complex 5, a hospital-associated lineage; 11 of 40 SCCmec/junctions evaded detection by BCID2 or Xpert. Variants had mecA instability and circulated in healthcare settings. Our findings reveal a unique escape mechanism and underscore a threat to diagnostic accuracy.

Podkowik et al. (2025) conducted an observational in Methicillin-resistant Staphylococcus aureus (MRSA) (n=2,432). CRISPR-Cas-associated SCCmec variants was evaluated on Evasion of rapid diagnostic detection (false-negative results by BCID2 or Xpert). CRISPR-Cas-associated SCCmec variants in MRSA evaded rapid diagnostic detection, with 11 of 40 variants escaping detection by BCID2 or Xpert, representing 2% of 2,432 screened patients.
