Hemodialysis improves hemostatic abnormalities in chronic renal failure by recovering platelet functions, specifically normalizing Ristocetin-induced aggregation through the removal of uremic toxins.
May support hemodialysis for uremic platelet dysfunction; observational data leaves open need for randomized outcome trials.
The purpose of this study was to investigate the effect of hemodialysis on platelet function in patients with chronic renal failure. In vitro study, the reduced aggregability of platelets in four patients with azotemia were found to be corrected by the addition of normal plasma or antihemophilic fraction in view of ADP- or Ristocetin-induced aggregation, but not by hemophilia A plasma. The effects of guanidinosuccinate, methylguanidine or urea nitrogen on Ristocetin-induced aggregation were examined by incubating them with platelet rich plasma obtained from five healthy subjects, and guanidinosuccinate showed a most inhibitory effect on platelet aggregation.Platelet retention and ADP- or Ristocetininduced platelet aggregation in fifteen patients were tested before and after hemodialysis. The rate of retention and the impaired aggregation of platelet were improved after hemodialysis. These results that the hemostatic abnormalities in chronic renal failure is improved by the recovery of platelet functions, since the interaction between ADP-or Ristocetin and platelet surface mediated with AHG complex is normalized due to the removal of uremic toxins by hemodialysis.

Isobe et al. (1977) conducted an observational in Chronic renal failure (n=20). Hemodialysis vs. Before hemodialysis (baseline) was evaluated on Ristocetin-induced platelet aggregation (p=<0.025). Hemodialysis significantly improved Ristocetin-induced platelet aggregation from 23.6% before to 41.2% after treatment in patients with chronic renal failure.
