頭頸部癌切除再建手術のリスク評価としてPOSSUMが有用性であるかを検討した。2010年1月から2011年12月までに頭頸部癌再建手術を行った188例を対象とした。POSSUMに基づいた予測式から予測合併症率を算出し,実際に生じた周術期合併症と比較した。実際に生じた周術期合併症は35例(19%)で認め,全身合併症17例(9%),SSI 20例(11%)であった。周術期合併症群と非合併症群に分けて比較した結果,POSSUMから算出した術後合併症率で有意差を認めた(p=0.01)。頭頸部癌切除再建手術のリスク評価の指標として,POSSUMは有用である。さらにROC曲線からYouden indexを用いて算出したPOSSUMのカットオフ値は45.9%であった。すなわち,POSSUMが45.9%以上の症例は周術期合併症発生に対しhigh riskな症例であるとすることができる。
