Prophylactic milrinone administration during off-pump coronary artery bypass grafting improves cardiac performance and mitigates hemodynamic disturbances associated with surgical manipulation.
Why the study?
OPCAB avoids complications of CPB but poses significant hemodynamic challenges, making it necessary to evaluate prophylactic milrinone as an adjunct to address these issues.
Does prophylactic milrinone improve hemodynamic stability in patients undergoing off-pump coronary artery bypass grafting?
Does prophylactic milrinone improve hemodynamic stability in patients undergoing off-pump coronary artery bypass grafting?
Prophylactic milrinone is a valuable adjunct in managing hemodynamic challenges during off-pump coronary artery bypass grafting, though careful titration is required to minimize risks of hypotension and arrhythmias.

May support adjunctive use in off-pump CABG but should not yet change practice; leaves open need for randomized confirmation.
Off-pump coronary artery bypass grafting (OPCAB) is a prevalent cardiac surgical technique designed to avoid the complications of cardiopulmonary bypass (CPB). Despite its benefits, OPCAB poses significant hemodynamic challenges, such as reduced cardiac output, increased atrial pressures, and mitral regurgitation due to mechanical shifts during surgery. Milrinone, a selective phosphodiesterase III inhibitor, has been proposed as a valuable adjunct to address these challenges. The present study provides a broad overview of the efficacy of prophylactic Milrinone administration in patients undergoing OPCAB procedures.

Padhi et al. (2025) conducted a review in Off-pump coronary artery bypass grafting (OPCAB). Prophylactic Milrinone vs. Placebo or other inotropes was evaluated. Prophylactic milrinone administration during off-pump coronary artery bypass grafting improves cardiac performance and mitigates hemodynamic disturbances associated with surgical manipulation.
