A 69-year-old female presented to the emergency department after overdosing on the antidepressant venlafaxine. In large ingestions venlafaxine can produce tricyclic antidepressant (TCA)-like myocardial sodium channel toxicity. The patient experienced significant cardiac interval QRS prolongation likely potentiated by metabolic acidosis following a seizure. This phenomenon is a well-described complication of sodium channel blockade and highlights the need for prompt intervention by emergency clinicians.
