Key result
Administration of dobutamine and atropine without pericardiocentesis successfully stabilized blood pressure and allowed for discharge at 96 hours in a dog with cardiac tamponade.
Why the study?
Does dobutamine infusion without pericardiocentesis improve survival in a dog with obstructive shock caused by cardiac tamponade following left atrial rupture?

Does dobutamine infusion without pericardiocentesis improve survival in a dog with obstructive shock caused by cardiac tamponade following left atrial rupture?
Dobutamine infusion without pericardiocentesis may be a viable treatment strategy for selected cases of obstructive shock due to atrial rupture in dogs.
May aid stabilization in canine atrial rupture tamponade; leaves open need for prospective validation.
We describe the successful management of an obstructive shock case caused by cardiac tamponade following atrial rupture. The dog was an 11‐year‐old female chihuahua with a 1‐year history of mitral valve insufficiency. During a radiographic examination, the dog collapsed. We diagnosed a case of obstructive shock caused by cardiac tamponade following left atrial rupture, secondary to mitral valve regurgitation. After administration of atropine and dobutamine, the blood pressure stabilised. The dog regained consciousness and was discharged 96 hours after collapse. The use of dobutamine should be considered in selected obstructive shock cases and may reduce the high mortality rate associated with atrial rupture.

Isayama et al. (2021) conducted a case report in Obstructive shock caused by cardiac tamponade following left atrial rupture (n=1). Dobutamine and atropine was evaluated on Blood pressure stabilization and survival. Administration of dobutamine and atropine without pericardiocentesis successfully stabilized blood pressure and allowed for discharge at 96 hours in a dog with cardiac tamponade.
