Tricuspid valve repair using artificial chordae is a feasible and effective technique following the resection of a rare tricuspid valve myxoma involving the papillary muscle and chordae.

成人原発性心臓腫瘍のうち粘液腫は最も多く，その75%が左房より発生し，右室特に三尖弁から発生するものはきわめて稀である．症例は33歳，女性で，脳動脈瘤の手術の際偶然心エコーにて心臓内腫瘍が発見された．心エコーでは大きさ14×12 mmの球状で，可動性のある有茎性の腫瘤で右室の腫瘍が疑われた．右室腫瘍で可動性のある場合は，弁口閉塞による血流障害から虚脱や失神発作を起こしたり，肺塞栓症も見られることから手術適応とした．手術は体外循環下に行われ，腫瘍は三尖弁の前乳頭筋より発生していた．腫瘍は乳頭筋と腱索とともに切除され，人工腱索による三尖弁形成術が行われた．病理診断の結果は心臓粘液腫であった．術後経過は良好で，術後15日目に退院した．術後5年目の現在腫瘍の再発もなく元気に日常生活を送っているが，完全切除後の再発の報告もあり，さらに三尖弁位での人工腱索の遠隔成績も不明であることから，今後もきめ細かい経過観察が重要である．
